SUBJECT: Request for New Additional Sense Codes

We recently came across some errors that could not be reported very
well because none of the existing error codes were close enough to
the problem we were trying to report.  Therefore I am requesting
that these error codes be assigned.


ASC ASCQ DTLPWRSOMC  DESCRIPTION

09h  04h DT  WR O    Head Select Fault


The above ASC and ASCQ field recommendations were made during
consultations with George Penokie and Larry Lamers to assure there
were no conflicts with other codes that have recently been accepted
for SCSI-3 but aren't shown in the SCSI-2 document.
